We are delighted to welcome Lorraine Hewie as our new Community Connector. We thought it might help to explain more about this relatively new post, which until recently was provided by Vicky Yuill.
What is a Community Connector?
At present Lorraine works from AMG’s surgery sites in Brodick and Lamlash. She works closely with the GP and Practice Nurse team to provide support and information on non-medical issues through a process called social prescribing. This aims to connect people to appropriate, community-based social activities which, in turn, support and benefit overall health and wellbeing.
Lorraine also works closely with North Ayrshire Council Services and the team at Arran Community and Voluntary Service in order that relevant local services are promoted, supported and signposted for our patients.
Lorraine also provides direct delivery of Smoking Cessation and Weight Management Programmes. Community Connector appointments are available by referral in Arran Medical Centres on Tuesday, Wednesday and Thursday mornings between 9:30am – 12:30pm. The length of appointment varies depending on which programme or service is required, these can be anything from 15 minutes to an hour in length.
How do I access these appointments?
The GPs and Practice Nurses will refer you to Lorraine with your permission if they think you would benefit from the skills that a Community Connector can offer. If you are keen for one of the Community Connector services then you can arrange an appointment (including by phone) with a GP or Practice Nurse to seek referral.
